Gather Health is a mobile platform that helps people with chronic diseases receive continuous care from their doctors and support from their family.

An individual views their treatment plan on a smartphone app, and records whether or not they completed each task. Doctors and other healthcare providers view this data through a web portal to monitor their patients’ progress, provide advice and encouragement, and adjust treatment plans as needed.
