    I’ve seen a few of these types of online services but unless they are providing some amazing design work – Google Apps Spreadsheet lets me get 8 of the 10 types of forms that Wufoo offer but for free with no limitations.

    The only significant issue that I’ve run into with the Google based forms, and I’ve heard from more than one person that had a similar issue, is that sometimes the forms fail to load, etc. It seems like this one is the leading alternative to using Google Apps. Personally, I’d either tend to use GA, or roll my own, but I’m just like that.
